• 縦隔・肺門部腫瘍・・・気管支腫瘍、気管支異物、リンパ節転移
• 肺血栓・塞栓症
• 大動脈解離
• 胸膜炎
• 肺実質病変・・・肺気腫、放射線治療後
• 炎症後の反応・・・Swyer-James-Syndrome
• 先天性肺動脈奇形
• 肺、心疾患の手術後、不適切なスワンガンツカテーテル留置

◆気管支の通過障害でperfusion defectを生じる理由

• その原因は諸説あるが・・・
  ・呼気時に腫瘍・異物がチェックバルブとして働き、air trappingすることで肺胞内圧が上昇し局所血流が低下する
  ・換気不全により反応性に低酸素性の肺血管収縮を引き起こす
• 本症例では肺野の透過性亢進に加え，呼気時の連続性ラ音を聴取しており、air trappingによるperfusion defectと考えられる
